Registration Overview

This single registration form is used to enroll in any GME Lifecycle Survey (Entry, Midterm, Exit, Alumni, and related modules). After submission, our Client Services team will follow up to confirm participation and coordinate next steps.

Important notes:

• A separate submission is required for each survey type.
• A separate submission is also required for each GME program (full-time, part-time/online, executive, specialized).
• Institutions participating in multiple programs and/or multiple lifecycle surveys should submit the form for each unique program–survey combination.

Only the Roadmap and Guidance Decision Packages include:

• Benchmarking and trending
• Up to three surveys per fielding year (for multiple intakes/outtakes)
• Student Loyalty Index
• Net Promoter Score (NPS)
• Key Driver Analysis
